Representative:Your Energy Specialist represents Energy Savers Network (ESN).
Workday Expectations: During the workday, we request that you secure all pets, do not smoke inside the home, and provide clear and unobstructed access to the water heater and air vents.
Interview Questions: The Energy Specialist will ask you about your home and family’s energy usage habits and history. We will use your past energy use to evaluate your home’s energy efficiency and provide solutions. We will keep your energy use history confidential.
Expert Opinion: Any verbal and/or written information we provide is based on observations and the professional judgment of ESN to help you save energy.
Release: Due to the number of factors beyond ESN’s control, ESN in no way represents or warrants a reduction in your energy bills. ESN expressly disclaims, and you release ESN from any and all liability for any damages of any nature (including direct, indirect, incidental, and consequential) arising in connection with participation in this program, including any follow- on measures performed by ESN or contractors.
Survey of Home: The Energy Specialist will survey areas and features of your home such as the garage, basement, attic, windows, doors, and most rooms and will develop a plan for measures to be installed. If there is an area of the home you do not wish to be surveyed, please advise the Energy Specialist.
Measures & Materials: Measures and materials installed by ESN will vary depending on the home. The Energy Specialist will demonstrate the use of these measures and ESN will install them for you with your approval.
Tenant Occupied Residence: Renters must obtain permission from the landlord through a separate agreement provided by ESN.
Eligibility & Signature: To participate in the program, you must be income-qualified. The homeowner or tenant must be present for ESN to complete the project and any follow up. An authorized signature at the bottom of this form is required before ESN can assess your home or install any measures.
Follow-up Services and Contractors. After an initial assessment and workday, ESN and I may agree on follow-up services such as home repairs, heat system repairs, insulation, appliance upgrades, etc. The terms of this agreement apply to those follow-up services. If a contractor performs the follow-up services then they are responsible for any warranty of their work or the materials installed.
Exchange Confidential Information: I authorize agents of ESN to exchange information related to the condition of my home, eligibility, and scope of services applied with partner organizations.
Photographic Release: I authorize ESN to take photographs during this project and to copyright, use, and publish such photographs in print or electronically.
Inspection Visits: I authorize ESN to make a post-installation inspection visit upon request for quality assurance purposes.
